For New Patients

Patient Forms
Please help us be prepared for your first visit by completing our patient information forms. Download, print, fill out, and bring with you to your first vist.
Your First Visit
At the first visit to the orthodontist, a clinical examination is done to determine if and when the patient is ready for treatment. Treatment options will be discussed, and the course of treatment will be recommended. If it is time to begin orthodontic treatment, an appointment will be scheduled to obtain orthodontic diagnostic records.
Your Records Appointment
Complete diagnostic orthodontic records will include plaster study models of the teeth, photos of the patient's face and teeth, and digital radiographs. This information will be used by the doctor to develop a custom treatment plan. These records are also helpful in tracking the progress of treatment as the teeth move under orthodontic care.
Consultation
After the doctor has analyzed the orthodontic records, a consultation appointment will be scheduled to discuss your treatment in detail.
Regular Appointments
The appointment to get your appliances (braces, expanders, XBow etc.) usually takes longer than the routine appointments. Our reception staff will let you know how long each appointment will be. You will then be seen at regular intervals of between 2 to 8 weeks. Longer appointments and emergency or repair appointments are scheduled during school hours when the doctor has extra time to spent with each patient. Our regular office hours are Monday to Thursday from 8:10 AM to 5:00 PM. Patients are seen by appointment only. We make every effort to be on time for our patients. This means we need you to be on time also. If you cannot keep an appointment, please notify us immediately. Cancel only if it is an emergency. It is difficult to reschedule you since most appointments are made 4 to 8 weeks in advance.
